Project Overview

The Stroke Community Rehabilitation (SCORE) initiative is a strategic community engagement project, established in partnership with **PERKIM**. Its primary goal is to transform and scale up an existing community center through Knowledge Transfer from academia to the community, prompted by SCORE's success in Kuala Lumpur.

The expansion to **Kelantan** began with a critical situational analysis in August 2019. This study, conducted at Hospital Raja Perempuan Zainab II (**HRPZ**) and HUSM, involved literature reviews, questionnaires, and in-depth interviews with healthcare providers.

**Identified Challenges:** Key barriers to effective rehabilitation were found, including patients' financial constraints, difficulties with transportation and parking, poor psychological state, limited hospital capacity, and a general lack of stroke rehabilitation awareness among patients.

**Customized Solution:** To improve compliance, the project focused on customizing the SCORE team for Kelantan. This involved planning a comprehensive rehabilitation unit, including physicians, nurses, various therapists (Physio, Occupational, Speech), support groups, counselors, and incorporating **traditional medicine** like massage and acupuncture. Concurrently, community engagement programs were delivered in collaboration with **Yayasan Orang Kurang Upaya Kelantan (YOKUK)** to educate the public on stroke risks and home exercises.

Project Outputs (Knowledge & Data)

  • Mini SCORE Workshops: Disseminated vital knowledge about **(1) stroke, (2) risk factors, and (3) home exercise methods** to the community.
  • Community Outreach: Participants of the survey were actively engaged and provided with stress balls.
  • Research Data (Qualitative): Produced a detailed report on the **barriers and needs** of stroke rehabilitation specific to Kelantan.
  • Research Data (Quantitative): Generated a report on patients' **Knowledge, Attitude, and Practice (KAP)** of stroke rehabilitation in the region.

Future Enhancement & Analysis

  • Finalization of the detailed **Knowledge, Attitude and Practice (KAP) score** for stroke rehabilitation.
  • Conducting a comprehensive **Thematic Analysis of FGDs** (Focus Group Discussions) to deepen understanding of patient experience.
  • Detailed **Feedback Evaluation** of the community engagement workshops to ensure continuous quality improvement of the intervention strategy.
